CONTEST RULES/HOW TO ENTER

• You can submit multiple entries for this contest drawing via the following methods:

1. During the month of August, bring *one* proof of entry to the county or state fair, for each appointment, per person, to Gallipolis Chiropractic & Wellness Center. Subsequent appointments will require a new proof of entry (to the fair) for each visit. Each fair proof of entry will give you one entry for the drawing. 
2. Leave a Google Review, to submit an additional entry for the drawing. You must submit the review at the time of your appointment. 
3. Refer a friend with a referral card and receive an additional entry for the drawing.
4. New Patients automatically receive an entry for the drawing.

• When visiting for your appointment, make sure to mention to our staff that you are participating in our contest.

TERMS, & CONDITIONS APPLY

This prize is not redeemable for cash. You must be 18 years or older to enter into the contest. Prior appointments cannot be attributed towards your entry balance.

ELIGIBLE LOCATIONS AND PERIOD

This contest and prize is only available to U.S. Citizens living within the Ohio, West Virginia, and Kentucky tri-state area. The contest submission period begins on August 1, 2022 at 12:01am, and ends on August 29, 2022 at 11:59pm.

THE WINNER

The winner will be chosen via a random drawing after the close of the contest period. Winners will be announced on Tuesday, August 30th 2022 . Multiple winners may be chosen. The number of gift basket prizes may increase for every 100 entries counted. If you are the winner, you will be notified privately. If you do not respond to our notifying message within two weeks of winning the prize, you forfeit the prize. The subsequent person drawn will be awarded the prize. 

HOW TO OBTAIN YOUR PRIZE

You must present a photo ID in person, to pick up the prize from Gallipolis Chiropractic & Wellness Center. The winner must pick up the prize from Gallipolis Chiropractic & Wellness Center within one month of being notified, or the win will be forfeited.
